Patient Support Programs

Dialysis is only one part of the care we provide. We understand that living with kidney disease affects many aspects of life — from nutrition to emotional well-being. That’s why we offer a range of additional services designed to support every aspect of your health. Our goal is to give every patient the tools, care, and guidance they need to live well and feel supported at every step of their journey.

Good nutrition plays a vital role in managing kidney disease. Our dietitians and nutritionists work directly with patients to build meal plans that support kidney health, manage symptoms, and fit real life — not just a generic diet sheet.

A well-functioning vascular access is central to safe, effective dialysis. Our vascular access programs support patients through assessment, placement, and long-term maintenance of catheters, fistulas, and grafts.

For eligible patients, a kidney transplant can offer a life free from regular dialysis. Our team supports patients through referral, transplant readiness education, and coordination with transplant centers.

Early detection saves lives. AHN runs regular community screening events to help people understand their kidney health and catch chronic kidney disease before it progresses.

For patients managing advanced kidney disease, our kidney supportive care program focuses on quality of life — symptom management, emotional support, and guidance for patients and families navigating difficult decisions.
